Why do teeth look the way they do?

In this case study the patient’s smile is due changes around the tooth.

The right lateral incisor has moved out of the bone to make it look longer than the adjacent teeth. This has caused more of the yellowish root to be exposed. The is very mobile and loose, and is causing pain on biting.

How does this happen?

Well, loss of bone also means loss of attachment. This happens in advanced periodontal disease. “Peri” means around and “dontal”, obviously means tooth. This occur where bacteria are left on the teeth to grow and eventual calcify leading to tartar or calulus formation on the teeth. Nastier germs hide under this calcified mess and cause the body to throw everything it has at it. Unfortunately, the immune system can’t remove the calcified gowth on the tooth, and actually harms the body, hence the loss of attachment and bone.

What to do?

We need to remove the tooth, as it is causing pain and ruining the bone level on the adjacent canine. Here’s the twist. Once we remove the tooth we can use it to improve the smile and achieve a more comfortable and healthier outcome.

We need to remove some of the root to allow the tooth to be placed at a higher level and allow the to bone to heal. The loose tooth was an irrantant on the body, so the inflammtion in the gums and bone will go once the tooth is removed.

Removing the tooth and shortening the root, will mean less root exposed.

The other issue to solve is the pain on biting. We adust the back of the tooth to ensure no pressure in the tooth.

Now for changing smile. The tooth is postion to improve the smile by bonding to the adjacent canine only, using tooth colour filling material.

There is no longer a loose tooth, no pain on biting, no long tooth nor exposed root. Flossing under the attached root is very easy. The gums grows aroung the root so it looks like a real tooth. If the gum shrinks aound the tooth, we can add more tooth coloured filling material.
